Client workflow
- Client completes the official PAR-Q+ and indicated follow-up.
- If medical clearance is indicated, client prints the form for their clinician.
- Client may book conditionally, agreeing to provide signed clearance by appointment start.
- Bio Blueprint independently reviews suitability and retains the right to modify or decline testing when safety protocols require it.
Physician clearance portal
Clinicians can download and print the clearance form and review the submission instructions. Patient-document uploading remains unavailable until the secure intake and private review workflow are verified.
